Your first visit to The PMR Center establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know our staff. Do We Accept your Insurance?

We participate in most major insurance plans including:

Aetna, Amerihealth, CHN, Cigna, Clover, Empire BC/BS of New York, HIP NY, Horizon BC/BS of New Jersey, Medicare New Jersey, Medicare New York, MVA, Oxford, Oscar, Qualcare, Secure Horizons, UHC, UHC Government, WC, 1199.

Please contact our office to verify that your provider participates in your particular insurance plan and product line.

Patients are always responsible for knowing their benefits and for payment of their co-pays, deductibles, co-insurance and all non-covered treatments.

• Your health insurance card
• All copies of medical records and imaging if you have had previous treatment for the injury for which you are being seen
• Payments for all co-pays, co-insurance, and deductibles (if applicable) will be collected when you check in on the day of your appointment
